What's the real upgrade policy?

i'm seeing mixed results when talk people iphone upgrades. more people , carriers in mix i'm curious real upgrade policy now.

in past, when next iphone available, current iphone owners less 18 months of service on contract given full discount upgrade latest iphone. i'm not sure true read people upgrading 3g 3gs , i4. did policy start , apply people owned original iphone?

what's deal? iphone faithful upgrade every year latest iphone when it's under 18 month minimum? if you've been able how? , don't mean upgrade pay more money. i'm talking full discount price.
 

if pay $100+ on line, line gets upgrade every year.
